Director of Performance Improvement Clinical Compliance

1.    Ensure that center locations and clinical programs are in compliance with all applicable

local, state and federal regulations

2.    In conjunction with the Chief Medical Officer and Director of Nursing, develops, updates and implements the following programs: Quality Assurance, HIPAA and Information Security, Infection Control, Clinical Protocols, Policies and Procedures,

3.    Ensure that employee training is provided consistent with applicable laws and regulations.

4.    Review, investigate, complete and corresponds with external audits and information requests.

5.    Develop and implement a patient comprehensive Risk Management Program which includes reviewing, training staff, tracking and trending incidents, HIPAA violation complaints, safety and adverse events

6.    Establish and perform monthly compliance self-audit checklists for pertinent NJ regulations governing SJFMC programs for all locations

7.    Ensure compliance with Health Start program

8.    Responsible for developing, coordinating and overseeing the health center’s Risk Management Program which includes but is not limited to ensuring employees are properly trained in Incident Reporting, HIPAA, Patient and Employee Safety, reviewing, tracking and trending patient satisfaction, patient complaints and incident reports

9.    Responsible for daily continuous oversight the Quality Improvement/Quality Assurance program, including ensuring the implementation of QI/QA operating procedures and completion of QI/QA assessments, monitoring QI/QA outcomes, and updating QI/QA operating procedures

10.Responsible for daily and episodic management and processing of claims-related activities while serving as the claims point of contact

11.Develop, review and revise Policy and Procedure to meet compliance with Standards for Licensure of Ambulatory Care Facilities NJDHS, OIG and HRSA. Evaluate ongoing compliance with Policy and Procedure

12.Ensure compliance with NJDHSS disease and mandatory local, state and federal reporting requirements

13.Develop and implement staff education programs consistent with state, federal and local regulations, including HIPAA, Information Security, NJ Ambulatory Care regulations, Risk Management and Infection Control

14.Ensure that audits are conducted in accordance with an approved schedule according to the centers’ Quality Assurance and Health Care plans

15.Represent the organization and participate in external meetings as required.

16.Acts as the primary contact for HHS and Department of Justice, in consort with the Director of HR, in the event of a lawsuit, responsible for timely and complete responses

17.Assists CMO with the PEER Review Program

18.Perform all other duties as assigned by the CEO

 POSITION REQUIREMENTS:

· Registered Nurse (RN)

1.    Function as a charge/lead nurse in all departments and the ability to delegate when appropriate.

2.    Provides case management by being a patient advocate, resource and contact person for patients.

3.    Provides basic education information to patients and family, focusing on self-care appropriate to their age and identified learning considerations and needs.

4.    Performs assessment data collection and documents subjective data (chief complaint and allergies) and objective data (vital signs/weight/etc.). Reviews basic health history information for patients; focusing on environmental, self-care and clinic discharge planning.

5.    Administers medications skillfully and correctly, and performs lab/diagnostic testing as ordered by provider and reports all critical lab values immediately.

6.    Demonstrates an ability to remain calm, perform effectively and professionally during peak periods of activity and emergency situations.

7.    Anticipates and prepares examination room with equipment and supplies required for the visit. Assists provider with examination and procedures per request.

8.    Utilizes universal precautions in obtaining specimens (venipuncture, cultures, etc.) and in delivery of patient care. Performs lab testing (as is required) including quality controls and maintains all laboratory/diagnostic test logs.

9.    Performs efficiently in stressful situations, seeking guidance, informing appropriate persons

10.and documenting events.

11.Assess nursing care needs of the patients such as triaging and assign Nurse duties to LPN, CMA and MA

Education and Experience:

 

-RN License

- At least 1-2 years of clinical experience in a medical office setting;

- Prior Community Health Center or ambulatory care experience preferred

Licensed Practical Nurse (LPN)

1.    Performs nursing procedures under the supervision of an RN and Nursing supervisor

2.    Collection of selected data about the health status of the patient

3.    Participate in the development of the plan of care with other team members and safely implements that plan.

4.    Communicate in an attentive and individualized manner in every encounter

5.    Verifies the patients understanding of the purpose of the visit

6.    Provide instruction/teaching to the patient according to the educational plan

7.    Document care provided or procedures performed accurately on patient records

8.    Provide each patient care in a safe, comfortable and private environment

9.    Provide preventive care information that matches patient age, sex and health status

10.Implement physician orders with the accepted nursing standards for license practical nurse

11.Perform and test control for CLIA waived test

12.Perform electrocardiography and venipuncture

13.Dispose of contaminated supplies and medical instrument

14.Authorize drug refills as directed by physician

15.Keeping waiting and examination room neat and clean

16.Implement physician orders with the accept standards

17.Review medical chart for appropriate laboratory and diagnostic test

18.Perform initial response screening approved by supervising physician

19.Prepare and maintain examination, laboratory and treatment areas

20.Make copies of patients records for other providers

Education and Experience:

 

-LPN License

- At least 1-2 years of clinical experience in a medical office setting;

Certified Medical Assistant (CMA)

Certified Medical Assistants are primarily responsible for assisting our Providers with patient medical care. Our Certified Medical Assistants report to the Nursing Supervisor as well as the Provider on-duty.

Responsibilities include, but are not limited to: - Excellent customer service and registering patients as well as handling related administrative duties; - Escorting patients to examination rooms; - Assisting with patient triage by taking medical history and vital signs; - Documenting relevant patient data in EMR; - Discharging patients with detailed instructions; - Maintaining efficient and organized patient flow; - Screens calls, schedules appointments, and calls patients as necessary;

- Tracking and replenishing inventories and other supplies; and - Assisting with equipment maintenance and ensuring cleanliness.

Education and Experience:

- CMA Certificate

- At least 1-2 years of clinical experience in a medical office setting;

- Prior Community Health Center or ambulatory care experience preferred;

- Team-player able to multi-task and work independently in a fast-paced environment;

- High-energy individual with a positive attitude; and - Comfortable working with electronic medical records (ECW).

Maintenance Worker

Duties/Responsibilities:

  • Cleans and assists with upkeep of the facilities.
  • Performs other related duties as assigned.
  • Keeps the grounds of the facility clean

Required Skills/Abilities:

  • Ability to follow instructions from supervisors or workers.
  • Knowledge of general repair.
  • Ability to use hand tools and power tools.
  • Excellent organizational and time management skills.

Education and Experience:

  • High school diploma or equivalent required.
